Definition

The reticulocyte test is a procedure used to count the number of reticulocytes, which are immature red blood cells, in the blood. An abnormally high or low reticulocyte count can provide insight into the function and health of the bone marrow.

Reticulocytes are produced in the bone marrow and are released into the bloodstream after 1-2 days of maturation. Over time, reticulocytes mature into fully developed red blood cells, which are essential for transporting oxygen throughout the body. Red blood cells have a lifespan of about 120 days, after which they are removed and broken down by the lymphatic system.

Abnormal reticulocyte test results may suggest impaired bone marrow function, excessive destruction of red blood cells, or a deficiency in iron. This test is helpful in diagnosing the cause of anemia and can be performed on both children and adults.

 

Indication

The reticulocyte test is useful for several medical purposes, including:

  • Diagnosing symptoms of anemia or chronic, hidden blood loss, such as fatigue, pallor, weakness, shortness of breath, and blood in the stool.

  • Confirming and evaluating abnormal results from a complete blood count, particularly when red blood cell, hematocrit, or hemoglobin levels are low.

  • Assessing the bone marrow’s ability to respond to the body's demand for red blood cells.

  • Determining the cause of anemia.

  • Monitoring the response to treatment in cases of iron deficiency anemia.

  • Assessing bone marrow function during chemotherapy or radiotherapy.

  • Evaluating bone marrow function following a transplant or graft procedure.

 

Contraindication

There are no specific contraindications for the reticulocyte test.

 

Preparation Before the Test

No special preparation is needed before the reticulocyte test. However, if you are taking certain medications, you should consult your doctor, as some medications can affect the number of reticulocytes in your blood.

 

Test Procedure

The reticulocyte test requires a blood sample, which is drawn from a vein. A small amount of blood, typically around 0.3-0.5 mL, is needed for the test, and it will be analyzed using a specialized instrument. Once the blood is drawn, you can resume your normal activities.

 

Normal and Abnormal Value

The results of the reticulocyte test are expressed as a percentage. The following are the normal values for reticulocytes in the blood:

As seen in the table above, normal reticulocyte levels vary according to age. The normal range lies between the upper and lower limits. Abnormal values are those that fall outside of this range.

 

Results and Recommendations (Follow-up Tests)

Normal Reticulocyte Levels

If your reticulocyte test results fall within the normal range, it indicates that the number of reticulocytes in your bloodstream is normal. This suggests that the bone marrow is functioning well and producing the mature red blood cells the body requires. It also indicates that there are no health issues, and you are in good health.

High Reticulocyte Levels

If your test results show a higher percentage of reticulocytes, it means that there is an increased number of immature red blood cells in your bloodstream. This can occur due to medical conditions such as excessive blood loss (due to bleeding) or hemolytic anemia (excessive destruction of red blood cells). In response, the bone marrow accelerates the production of new red blood cells, releasing them into the bloodstream in an immature form, leading to an increased number of reticulocytes.

However, high reticulocyte test results alone do not definitively diagnose any specific condition. To determine the cause of the elevated reticulocyte percentage, your doctor will typically recommend additional tests, such as a complete blood count (CBC), iron profile, peripheral blood tests, and red blood cell counts.

Low Reticulocyte Levels

A low reticulocyte count, meaning fewer reticulocytes in your bloodstream, suggests that the bone marrow is not producing red blood cells as efficiently as it should. This can be due to various medical conditions, including:

  • Iron deficiency anemia

  • Vitamin B12 or folate deficiency anemia

  • Aplastic anemia (where the bone marrow fails to produce enough red blood cells)

  • Bone marrow damage or disorders

  • Cirrhosis

  • Kidney disease

  • Erythropoietin deficiency (a hormone that stimulates red blood cell production in the bone marrow)

To diagnose the exact cause, your doctor may recommend additional tests such as an iron profile, complete blood count, bone marrow biopsy, vitamin B12 and folate level tests, peripheral blood tests, erythropoietin level test, or kidney and liver function tests.
