How It Works

Topical acyclovir is an antiviral medication that helps treat painful, fluid-filled blisters caused by herpes simplex virus infections, including those on the lips (herpes labialis) or genitals (herpes genitalis). Once applied, acyclovir is converted into acyclovir triphosphatase by enzymes in the body, which then inhibits the virus’s ability to replicate and spread.

 

Indications

Topical acyclovir is used to treat infections caused by the herpes simplex virus, particularly herpes labialis (cold sores on the lips), or herpes genitalis (genital herpes).

 

Contraindications

Topical acyclovir should not be used in individuals with a history of hypersensitivity or allergic reactions to acyclovir, such as hives or swollen eyelids. 

 

Side Effects

Side effects of topical acyclovir are generally rare, but some people may experience local burning sensation at the application site, hives, itchiness, peeling skin, rashes, swelling, pain or other signs of irritation. If any of these symptoms persist or worsen, consult a doctor.

 

Types

Topical acyclovir is available in the following forms:

  • 3% Acyclovir eye ointment

  • 5% Acyclovir skin ointment or cream

 

Dosage

Genital herpes or herpes labialis

Adult dosage:

  • The 5% cream is applied to the affected skin area 5 times per day, every 4 hours, for 4-10 days.

  • The 5% ointment is applied to the affected skin area 6 times per day, every 3 hours, for 7 days.

Pediatric dosage:

For the 5% cream, the dosage is the same as the adult dosage.

 

Safety

Pregnancy:

Topical acyclovir is classified as FDA Category B, meaning animal studies have not shown fetal abnormalities, and there are no well-controlled studies in pregnant women. The use of this medication is considered likely safe during pregnancy but should be discussed with a doctor.

 

Drug Interactions

Currently, no significant clinical reports indicate major drug interactions with topical acyclovir. However, if using other medications concurrently, it’s recommended to consult with a doctor to ensure proper monitoring.
