How It Works

Phenylephrine is a medication commonly used to alleviate nasal congestion caused by infections or allergies. It works by constricting the blood vessels that become enlarged due to infection or allergies, which helps relieve the feeling of stuffy nose. Additionally, phenylephrine is used to dilate the pupils for eye examinations.

 

Indications

Phenylephrine is used to treat nasal congestion and to induce pupil dilation (mydriasis) before eye exams.

 

Contraindications

Phenylephrine should not be used by individuals with severe hypertension, ventricular tachycardia (rapid heart rate), or severe hyperthyroidism (excess thyroid hormone).

 

Side Effects

While side effects are rare, some may occur, including:

  • Irregular heart rhythm

  • Nausea and vomiting

  • Headache

  • Tremors

  • Weakness

  • Anxiety

  • Insomnia

  • Shortness of breath

  • Blurred vision

  • Eye pain or irritation

 

Types

Phenylephrine is available in the following forms:

  • Tablets

  • Syrup

  • Eye drops

 

Dosage

For relieving nasal congestion

Adults and children >12 years old dosage: 10 mg every 4 hours, up to 7 days. The maximum daily dose should not exceed 60 mg.

For dilating the pupils

Adult dosage: 2.5% eye drops, 1 drop in each eye about 15–120 minutes before the eye examination.

Children: Same dosage as adults.

 

Safety

Pregnancy:

Phenylephrine is classified as FDA Category C (animal studies have shown potential harm to the fetus, but no well-controlled human studies exist). Therefore, the potential benefits and risks should be weighed carefully, and it should be used cautiously during pregnancy.

 

Drug Interactions

When used with tricyclic antidepressants, phenylephrine may enhance the effects of the antidepressant. Special care and supervision are advised when taking these medications together.
